In the above correspondence, Fixe et al. reported macrophage colony-stimulating factor (M-CSF) levels in the plasma of bone marrow aspirates in several hematological malignancies [1]. They reported significantly higher BM plasma M-CSF levels than in blood plasma. They also observed significantly lower activities in patients with multiple myeloma. We measured granulocyte colony-stimulating factor (G-CSF) levels in the plasma of bone marrow aspirates [2]. Many hematopoietic cytokines are believed to act locally as paracrine elements, and blood plasma concentrations are generally low. We agree that measuring those cytokine levels in BM plasma rather than in blood plasma may more directly reflect the pathophysiological mechanism of disordered hematopoiesis. We do not know the reason for the difference between M-CSF and G-CSF levels in multiple myeloma patients, in which we observed significantly higher G-CSF levels. Our result is in agreement with that by Portier et al., suggesting that IL-1, IL-6 and G-CSF, but not GM-CSF, could play a role in the development of myeloma disease in vivo [3]. Still, the c-fms proto-oncogene, which encodes the receptor for M-CSF, had been expressed in hairy cells, but not in myeloma cells [4].
